Grote controle 1 jaar na mijn stamceltransplantatie.
Voorbeeldpatiënt?
Vele lezers komen via Google bij dit blog uit. Daar zitten ook veel lotgenoten bij. Voor hen is het interessant om te weten of mijn beschreven leven als patiënt met myelofibrose (MF) een voorbeeld is voor het leven van iedere MF patiënt. Het antwoord is duidelijk: Nee! Ook de behandeling en de eventuele complicaties verschillen enorm per patiënt. Een van de verpleegkundigen die we gisteren spraken in het ziekenhuis, (en zij spreekt veel patiënten met MF en een stamceltransplantatie), noemde me een medisch wonder en dat geloof ik gelijk.
Natuurlijk heb ik duidelijk beschreven wat het leven met MF inhoudt. Maar niet iedere MF patiënt krijgt een vleesetende bacterie of acute leukemie. Wel is de les die geleerd kan worden uit mijn verhaal, dat als je MF hebt in een ver gevorderd stadium (high risk) met een lage weerstand, je goed de extra strenge hygiëne richtlijnen moet volgen (dus niet zoenen, contacten met zieke mensen vermijden, etc.)
Eerlijk gezegd heb ik vrijwel alle complicaties gekregen die je als MF patiënt kunt krijgen, en daarom ben ik wel trots dat dit blog een volledige beschrijving is geworden voor het leven met MF, een vleesetende bacterie, acute leukemie en een stamceltransplantatie. Mede dankzij mijn vrouw Marjolein die de moed en de kracht had om het blog dagelijks bij te werken, toen ik daartoe niet in staat was.
Grote controle.
Gisteren bijna de hele dag in het ziekenhuis doorgebracht. Begonnen om 9 uur met een gesprek met een verpleegkundig specialist. Uiteraard het langste stilgestaan bij de vermoeidheid. Hij was blij dat het vitamine D probleem was opgelost (de nu gemeten waarde is normaal) en na het nodige overleg denkt hij dat het een combinatie is van een aantal zaken zoals medicijnen, te hoog ijzer, chemo.
Hij is ook de eerste die zegt dat de grote hoeveelheid prednison die ik slik en geslikt heb kan zorgen voor botproblemen. We hadden dit al gelezen en zijn erg benieuwd naar de uitslag van dat onderzoek dat ook vandaag plaats vindt. Uit onderzoek blijkt dat 25% van de patiënten met chronische afstoting (en die daartegen grote hoeveelheden prednison slikken) een kunstheup hebben.
Bloeduitslagen.
Vorige week 15 buizen bloed laten prikken en die uitslagen zijn allemaal binnen. Uitgeprint neem ik ze mee en we bespreken ze.
Het hoge ijzer (ferritine =4.216 in plaats van maximaal 250) gaan we nu oplossen. Het is ontstaan doordat ik 70-80 bloedtransfusies heb gehad met daarin ook ijzer van de bloeddonor. Dit ijzer wordt niet door mijn lichaam verwijderd en kan zich afzetten op de lever. Waarschijnlijk zit ik de komende 2 jaar iedere maand een keer in het ziekenhuis om een halve liter bloed te “aderlaten”. Heel raar idee. Eerst loop ik er 1,5 jaar rond om heel regelmatig 2 zakken bloed te halen omdat mijn Hb. te laag is, nu kan ik het weer terug brengen. Afijn, als het maar helpt.
Voor het eerst zie ik ook zwart op wit mijn nieuwe bloedgroep: A+, de bloedgroep van de donor en dat terwijl mijn oude bloedgroep O+ was.
Mijn cholesterol is te hoog. Niet gek als je je realiseert dat aankomen mijn hoogste prioriteit was. 20 kg in 20 weken erbij eten krijg je niet voor elkaar met enkel gezonde voeding. Oeps, dat wordt even diëten voor Dolf.
De nierwaarde is achteruit gegaan. Enige dat ik hieraan kan doen is meer drinken. Het afbouwen van de Neoral (dat hier ook een oorzaak van kan zijn) kan nog niet i.v.m de afstoting van huid, mond, longen en ogen. De leverwaarden zijn bijna weer goed.
Inentingen.
Ook besluit hij na overleg met mijn hematoloog om het inentingstraject op te gaan starten.
Ik loop nu onbeschermd, als een baby, zonder weerstand tegen kinderziekten rond. Bij mijn bezoek begin juni krijg ik een schema met daarin de vaccinaties tegen kinderziekten. Er was nog wat twijfel of ze goed werken met de Neoral samen, maar kennelijk wel. Verder oefeningen voor mijn mond gekregen omdat de afstoting in mijn mond ook voor bindweefselvorming kan zorgen en de spieren kan doen afnemen.
Om 10 uur afspraak bij de stamceltransplantatieverpleegkundige. Daar komt weer mijn uitputting aan de orde, maar ook allerlei andere zaken zoals acceptatie van de ziekte door mijn omgeving. Conclusie is wel dat dit geen gebroken been is….. 3,5 jaar geleden werd de diagnose MF gesteld. Er is een zware weg afgelegd, maar er kan nog wel een revalidatie proces van 2-5 jaar volgen. Het is voor velen om ons heen niet goed voor te stellen wat dit alles voor ons in het dagelijkse leven inhoudt.
De verpleegkundige geeft aan dat de vermoeidheid ook kan komen doordat mijn longen langzaamaan minder zuurstof zijn op gaan nemen. Gelukkig staat er voor vandaag een longfunctietest op het programma dus daar komen we dan wel achter.
HR-CT van de longen.Direct daarna door naar de röntgen voor een hoge resolutie-CT van mijn longen. Ik herken het apparaat nog van vorig jaar. Toen werd ik er, terwijl ik aangesloten was op allerlei infusen, in mijn bed naar toe gereden en o, wat was ik toen zwak. Nu klim ik met gemak op de tafel en onderga het onderzoek dat circa 10 minuten duurt.
Koffie.
Nu tijd voor een beloning in de vorm van koffie en een broodje. Ik voel me beroerd door het drukke programma en zou het liefste even slapen, maar dat gaat natuurlijk niet. Spijt dat ik het niet over 2 dagen heb laten verdelen maar ja, toen de afspraken in februari werden gepland ging het een stuk beter met me. Even overweeg ik om de onderzoeken die in de middag staan gepland te laten verzetten naar een andere dag, maar na 2 paracetamol knap ik weer op en gaan we naar het lab.
Bloedprikken.
Ja, op het lab kennen ze me ondertussen wel. Alweer? Ja, vorige week 15 buizen afgenomen en nu 10. Totaal dus 25 in een week! Als ze maar vinden waardoor ik zo moe ben, vind ik alles best.
Hartfilmpje
Door naar de afdeling voor een ECG. Het hartfilmpje, de zoveelste in 2 jaar, is snel gemaakt. De laborante ziet dat het is voor de 1 jaarlijkse controle na een stamceltransplantatie en ze vraagt belangstellend hoe het gaat. We kletsen wat en ze blijkt toch veel kennis te hebben van een transplantatie. Leuk. Vooral de opmerking “ik moet toch even wat haren wegscheren van uw borst omdat de plakkers anders niet houden”, “ik realiseer me dat ze net nieuw zijn, het spijt me”.
Longfunctie.
Een deur verder zit de longfunctie. Die moet getest worden, maar de afspraken sluiten niet goed aan. Ik vraag of ik eerder mag komen en dat is geen enkel probleem. Al snel zit ik in een glazen kast met een knijper om mijn neus in een buis te blazen. In-uit hard-langzaam. Geduldig volg ik de commando’s op. Ben benieuwd naar de uitslag. Zuurstof tekort zou immers ook een oorzaak van de vermoeidheid kunnen zijn.
Koffie.
Met een voetbalvriend die in het UMCU werkt, drinken we tussendoor even een kop koffie en kletsen gezellig bij. Niet alleen over mijn ziekte, maar ook over zijn vrouw die afgelopen Koningsdag terecht een lintje heeft gekregen!
Botdichtheid.
Tijd voor het laatste onderzoek. Ze gaan kijken of ik botontkalking heb door het veelvuldig en in hoge dosis gebruiken van prednison (dat ik slik tegen de afstoting).
Dit is voor mij een nieuw onderzoek en ik ben benieuwd hoe het in zijn werk gaat. In de dexa kamer moet ik gaan liggen op een apparaat. Over mij heen gaat een soort camera die de dichtheid meet van de botten in mijn heup en van mijn wervelkolom. Ik heb best last van mijn heupen en ben benieuwd naar de uitslag.
Vriendelijke UMCU medewerkers!
Dit was toch een dag ziekenhuis waarbij we heel erg veel medewerkers op heel veel afdelingen hebben gesproken. Wat iedere keer weer op valt is de vriendelijkheid waarmee ze ons helpen. Geen een van de medewerkers die ik tref in het UMCU is chagrijnig. Complimenten aan de HR afdeling die hier ongetwijfeld op zal letten.
Het meeste contact hebben we met de afdeling hematologie en omdat het de dag van de verpleging is (en vrijdag mijn 1 jarige verjaardag) geven we ze een paar potten engelse drop. Heel leuk om een aantal van “mijn” verpleegkundigen weer terug te zien. Tjonge wat hebben ze hard moeten werken om me er bovenop te krijgen en o, wat waren ook zij blij om ons weer te zien.

i have tried all that stuff, running anti virus (Mc afee), changing setting through gpedit.msc etc, but then this problem “task manager has been disabled..” keeps appearing everytime, even when the subject setting in group policy is “disabled”. Forgot to write that the delamination could be caused by ingress of moisture and subsequent frosts. Putting it down to excessive wind (overload) without a detailed investigation is IMHO negligent.
